Chinese Medicine Treatment (60-min)

For new and current patients, please select this option to make an appointment. For new patients wanting to bill insurance, please a) call the member service line at the back of your insurance card, b) verify whether you have acupuncture benefits, c) confirm whether Dr. Michelle Yan, DSOM, LAc., (Path Solaris LLC) is paneled/credentialed as an in-network provider. If the answer is no, ask if your insurance pays for out-of-network provider visits, d) check whether you have a deductible, and whether you require pre-authorization from your physician, e) check how many acupuncture visits you get in a year, and what your co-pay is. Once you have these information, and are good to go, please schedule an appointment, and email a photo of the front and back of your insurance card and driver's license to Ms Sara Danko ([email protected]). Sara will respond to your email and help us get started with the billing process.
