Substance Use Disorder Assessment

Select this option to schedule your substance use disorder assessment via zoom. All substance use disorder assessments are completed via zoom. Please note your credit card will NOT be charged upon booking and serves as your means of confirming your appointment. A staff member will reach out via phone to gather and confirm demographic information and process payment over the phone. A zoom invitation will be sent your email address the morning of your scheduled appointment. ALL INTAKE PAPERWORK MUST BE RECEIVED BY OUR OFFICE AT LEAST 24-HOURS PRIOR TO YOUR SCHEDULED APPOINTMENT IN ORDER FOR YOU TO BE SEEN.

OWI Assessment

Select this option if you have been charged with and/or convicted of an OWI offense and to complete your assessment via our HIPPA compliant tele-health platform. All OWI assessments are done via HIPPA compliant telehealth. Please note your credit card will NOT be charged at the time of booking and will only be charged when you appointment is accepted and confirmed by our office. A staff member will reach out phone or email to gather and confirm demographic information. A zoom link will be sent to your email address the morning of your scheduled appointment. ALL INTAKE PAPERWORK MUST BE RECIEVED BY OUR OFFICE AT LEAST 24-HOURS PRIOR TO YOUR SCHEDULED APPOINTMENT IN ORDER FOR YOU TO BE SEEN.

Krista is a licensed clinical social worker in Iowa. Krista has been providing client-centered, compassionate care to individuals in need of substance use disorder assessment and mental health therapy for close to 20 years. She has specialized knowledge in EMDR techniques and assists clients with recovery from trauma, addictive/compulsive behaviors and other mental-health issues and symptoms.
