YOU WILL BE PAYING A $100 DEPOSIT ONLINE TOWARDS THE TOTAL TRIP PRICE WHICH CAN BE PAID IN PERSON WITH CASH OR CREDIT CARD. A 3.5% PROCESSING FEE WILL BE APPLIED TO CREDIT CARDS CHARGES. BY PAYING THE DEPOSIT YOU AGREE THAT YOU HAVE READ OUR LIABILITY RELEASE / RELEASE OF IMAGES. I, the undersigned, wish to contract Cocoa Beach Surf Fishing Charters, LLC for purposes of taking me Surf Fishing. By signing below, I am accepting the responsibilities and potential dangers associated with participation in this Surf Fishing activity. I realize there are certain risks inherent in my participation in this particular activity including but not limited to: Sun burn/poisoning, tripping hazards, fish bites/stings, slipping, falling, and injuring joints/bones and injury and/or death by lighting or rip currents and other dangers associated with the beach and water. I also realize that it is my responsibility to engage in physical activities only to the extend that I am physically able. I further state for the record that I have no such medical conditions that would prevent me from surf fishing. With this in mind, I hereby voluntarily assume all risks of whatever nature associated with or resulting from my contracting Cocoa Beach Surf Fishing Charters, LLC. I discharge, relieve, and hold harmless Mr. Lukas J Brickweg Owner/Manager of Cocoa Beach Surf Fishing Charters LLC and all of its officers, agents and employees, acting officially or otherwise, from any and all claims, demands, action or cause of action on account of death or personal injury of the undersigned or on account of the loss or destruction of personal property, which may occur as a result of undersigned’s participation in the Surf Fishing Trip. I hereby give permission for my transportation to any medical facility or hospital and I authorize any guide or medical personnel to render necessary medical care for myself. I, therefore, freely and voluntarily executed this release of my own free will for my family, my minor children, my heirs and executors and myself. I have read, understand and acknowledged the risks and liability for myself participating in this Surf Fishing program.
