1st Session Individual Insurance-Pay Session

Please book 1 week out so that we can set up your insurance benefits and complete the insurance verification request form at https://www.theresarodriguez.com/client-verification. Then you will receive a follow up email from my billing company Alma that will let you know what your copay and deductible are. This visit includes getting to know you and understanding your needs (an assessment) and beginning to provide some possible therapeutic ideas.

EAP Counseling

This is for ongoing clients for individual therapy (unless previously seen). If you are a new client please choose “1st Session EAP “ option. Please be aware that because the EAP will not allow me to bill canceled appointments, your appointment reservation fee is not reimbursed UNLESS cancelation occurs within 24 hours of your appointment. This enables me to fill the spot you reserved if you cannot make your appointment. Please contact me if you or your minor child has a doctor’s note or some other significant unforeseen issue. Telehealth service at the time of your appointment is available to help preserve an appointment and avoid the fee. Thank you!
