New Patient Exam and Cleaning - for all ages. When booking, please let us know your child's name and DOB. To complete your booking, please look for an email that includes our new patient forms. If you will be using dental PPO insurance we will need that information as well. We currently do not accept any HMO plans. If you have a question about your insurance, please give us a call. If a specific day/time is needed and you don't see it here, please give the office a call.
