The Front-PT Evaluation for Members
**SELECT THIS SERVICE IF THIS IS YOUR FIRST VISIT AND YOU ARE A MEMBER OF THE FRONT CLIMBING CLUB** This includes: review of past medical history, thorough physical examination, discussion of findings, treatment, and an individualized corrective exercise plan. This includes a $10 discount.
The Front-PT Evaluation for non-members
**SELECT THIS FOR YOUR FIRST VISIT IF YOU ARE NOT A MEMBER OF THE FRONT CLIMBING CLUB** This includes: review of past medical history, thorough physical examination, discussion of findings, treatment, and an individualized corrective exercise plan. The Front-PT Follow up for non-members
**SELECT THIS AFTER YOU HAVE PARTICIPATED IN AN EVALUATION IF YOU ARE NOT A MEMBER OF THE FRONT** This includes: Review of your exercises, discussion regarding progress, treatment, progressions for exercise program
Cupping/Needling
This includes a brief discussion regarding past medical history and current medications to ensure appropriateness. Additionally, a truncated evaluation to determine which tissue will be treated and a test-retest to assess your response.
Virtual Visit: Evaluation
Virtual PT visit via Doxy.Me with your physical therapist. Select this option if this is your FIRST visit for your specific complaint. You will receive an intake form to better inform your PT and an invite prior to your scheduled visit to enter the virtual "waiting room."
Virtual Visit: Follow Up
Select this option if you have already participated in a PT evaluation and want to follow up with your PT. You will receive a Doxy.Me invite at the time of your visit to enter the virtual waiting room.
Injury Screen
Book this if you have a more acute(4 weeks) injury. Longer duration injuries will require a more thorough evaluation. This includes a 20 minute evaluation and discussion regarding the exam findings and guidance regarding next best steps.
