Welcome to Rochester Pain & Performance! During this session with Dr. Matt, we will go through your history, complete a physical examination, and begin treatment to address what brought you to us. Once this appointment is scheduled, we will send an email containing the new patient paperwork. We kindly request that you print and bring the completed forms with you to your appointment.
