New Client Consultation (indicate due month in notes)

Schedule an initial consultation to learn more about homebirth and the services I offer. Both the pregnant woman AND her partner are required to attend. Please indicate your due month in the notes when you make an appointment. I have a limited number of spots for each month, and frequently there is a waiting list. Spots are filled on a first come, first serve basis and a consultation is not a guarantee of a spot on the calendar. The consultation fee is not refundable.

The initial prenatal visit usually takes a bit longer, and there is a $500 payment due at this visit. You will soon be receiving an invitation to fill out your intake information; please make sure this is filled out completely prior to your appointment.
