1 Standard Nutrition Consult
⎈Nutrition Counseling customized for your personal health needs ⎈You gain evidence based principles to get and keep your numbers in healthy range ⎈Personalized food recommendations targeted to develop your health quality ⎈Food Analysis to expose deficiency (3-day food diary must be submitted​ at least 24 hours prior to appointment arrival)
1A Standard Nutrition Follow Up
⎈Assess progress toward goal weight and metabolic change. ⎈ Adjust nutrition care plan based on your progress toward goals ⎈ Determine intensity and improvement in quality of outcomes. ⎈ Review pertinent lab reports/other objective data during follow-up. ⎈ Bring all requested documentation to follow up for thorough analysis
2 Diabetes Nutrition Consult
⎈ Recommendations per ADA standards of care. Calculate BMI at every session ⎈ ADA recommends diet, physical activity, behavioral therapy designed to achieve more than 5% weight loss ⎈ High intensity interventions: ADA recommends engaging in more than 16 sessions within 6 months all disciplines ⎈ Individualized diet, low calorie strategy for short (3 months) and long term (12 months or more) goals planning ⎈ Weight monitoring regularly is ADA recommended.
2A Diabetes Follow Up
⎈ Life style management, to support eating patterns that encourage intake of nutrient dense food to improve health status ⎈ To achieve and maintain body weight at healthy goal; calculate current BMI at each session; per ADA recommendation ⎈ Identify each of your specific nutrient deficiencies and provide recommendation to correct them ⎈ Provide and adjust strategy and tools that promote health and retard disease progression or most importantly, prevent disease from occurring in the first place.
3 BMI Nutrition Consult (Weight)
⎈ To achieve and maintain body weight at healthy goal; calculate current BMI at each session ⎈ Diet, physical activity, and behavioral therapy designed to induce more than 5% reduction in body weight for individuals with diabetes type 2 ⎈ More than or equal to 16 such intervention sessions in a 6 month period, to induce 500-750 calories deficit in per day. Short term goal is considered to be 3 months. Long term goal is greater than 12 months ⎈ Diet composition designed to prevent nutrient deficiency during weight loss period.
3A BMI Follow Up (Weight)
