PLEASE NOTE THIS IS ONLY A REQUEST. DO NOT CONSIDER YOUR APPOINTMENT BOOKED UNTIL YOU RECEIVE A CONFIRMATION. Due to COVID-19 appointmnents are spaced out to allow for surface cleaning and to allow for social distancing between patients. In order to effectively manage our safety my availability is much less than typical. AS A REMINDER TO CURRENT PATIENTS I AM AN OUT-OF-NETWORK PROVIDER AND I DO NOT SEE MEDICARE, MEDICAID, WORKERS COMPENSATION OR NO-FAULT CASES. IN FURTHER, I DO NO BUSINESS WITH MANAGED CARE ORGANIZATIONS, AS WELL AS ANY PLANS THAT REQUIRE AUTHORIZATION OR PRE-AUTHORIZATION. Effective 3/1/2021 if you have a commercial medical insurance plan with out-of-network benfits you will be responsible for submitting claims. I will provide you with the necessary CPT codes and ICD 10 codes upon request. If you have not been seen in the office for more than 30 days you may have to be re-evaluated. Re-evaluations may be more difficult to accomodate.
